The consumption of alcohol by the second officer on board the general cargo vessel Scot Carrier led to his distraction while manoeuvring the vessel. But safety failings ran much deeper than that
The accident report into a collision in which two Danish seafarers died also found evidence that disabling of safety barriers had become normal behaviour on the operator’s ships
