Flag states fail to file serious casualty reports

Of almost 900 very serious maritime casualties recorded in IMO’s database over the last five years, just over half are the subject of filed mandatory investigation reports, writes Nidaa Bakhsh. It means no lessons can be learned from 47% of cases and no corrective measures taken, defeating the point of the exercise

In the past four years nearly 50% of very serious casualty reports that should be submitted to the International Maritime Organization are still missing
